1. Obtain documentation forms from the DSC website at http://www.disability.uci.edu/forms/ . Submit forms to your healthcare professional for completion then return to DSC via mail, hand delivery or fax at (949) 824-3083.

2. Schedule an appointment to meet with a counselor:

Hearing & Visual Impairments	Erin Pinkus
Learning Disability and ADHD	Amy Cramer or Dr. Jan Serrantino

Psychological and Acquired Brain Injury	Greg Ferrer, M.A.
Temporary Disabilities	Erin Pinkus or Amy Cramer

3. Meet with your counselor to officially register and discuss your needs.

4. Attend a DSC Orientation

5. Meet with your DSC counselor at least one time each quarter

6. Updated documentation may be required during your career at UC Irvine
