REQUESTING OF ACCOMMODATIONS AND/OR SERVICES

ONLINE Accommodations Request Form (ARF)- (available 10th Week Fall 2009) for requesting faculty letters, notetakers and/or captioners. Also view the accompanying policy.
ONLINE Notetaker Request Form (NRF) - for registered DSC students eligible to receive notetaking as an accommodation.
ONLINE Notetaker Application Form (NRF) - for applying to be a notetaker for students with disabilities.
